ListView - Creating Column Headers

I followed your instructions step by step and nothing happens to the form.  I did not see any change in my form.  There were no column name DATE.  Could you send me a step by step instruction from the start to the end without skipping any steps to create a LISTVIEW panel with two columns.  Like below

DATE                  DESCRIPTION

The date column would be name ddate and the description would be name ddesc
